This wouldn't actually be that disruptive for gameplay. You could always adopt a child and make him your heir. Still, anyone who cares about genetics or lineage would be severely disappointed.

Caspian has said that he doesn't intend to include adoption, only blood relatives can become heirs (and maybe spouses? I can't remember). This is so that people who start as wards can't gain the system and get the bonus skill progression, then immediately be adopted and gain the advantages of having a family.
I think a disease that prevents you from having children is unlikely because of the way the inheritance system is set up.
